Patient-Centered Medical Home (PCMH) Congress is organized by Healthcare Made Practical (HMP) Communications LLC and will be held from Sep 14 - 16, 2018 at San Diego Convention Center, San Diego, California, United States of America.
The target audience for this medical event is professionals devoted to transforming care through the medical home model of care, including physicians, allied health professionals, practice administrators, quality managers, and consultants. This CME Conference has been approved for a maximum of 21 AMA PRA Category 1 Credits.
Conference Description:
