Tammy Daniels has spent more than 20 years serving in various roles in healthcare, including primary care and specialty care operations, quality improvement, human resources, and learning and development. She's finally found her "dream job" in the office of Value-Based Healthcare for Baylor Scott and White Health. In her current role, she works to help clinic sites transform their practices and negotiate the Patient-Centered Medical Home application and recognition process. The toughest (yet most rewarding) period of her career was the time she spent assisting 51 clinic sites achieve Level 3 PCMH recognition in less than one year.
Her other areas of interest include team strategic planning and goal-setting. Current areas of focus include preparing more than 70 primary care sites to achieve/renew their PCMH recognition under the 2017 standards and supporting three specialty sites who have achieved Level 3 Patient-Centered Specialty Practice (PCSP) status. Tammy has a Master of Business Administration degree, a Professional in Human Resources certification, and has also earned PCMH Certified Content Expert credentials.
