Susanne Campbell BS, RN, MS, PCMH CCE

In 2013, Susanne Campbell, RN, MS, PCMH CCE, joined the Care Transformation of Rhode Island multi-payer initiative as a PCMH content expert and Senior Project Manager, having previously assisted primary care practices in various settings achieve NCQA Patient Centered Medical Home (PCMH)Recognition. This “hands on” PCMH experience and clinical operations background have been valuable assets in assisting primary care practices improve care. Susanne has a broad range of clinical and administrative experiences in outpatient primary care practices, community health centers, home care, long term/assisted living care.
In her current CTC-RI position Susanne has supported the expansion of adult and pediatric primary care practices with achieving NCQA recognition, improving clinical and customer experience outcomes and decreasing total costs of care. An important aspect of her work within CTC-RI has been working with nurse care managers, health plans and practice reporting staff in developing a care coordination population health framework for working with high risk and at risk patients, and implementing behavioral health and community health teams in primary care. Susanne received her Master in Gerontology Nursing from the University of Rochester, her Masters in Health Service Administration from Salve Regina University, and her BSN from Mount Saint Mary College in Newburgh, NY.
