Patrick W. Page MD

Patrick Page has been a family physician for 35 years in Grand Junction, Colorado. Dr. Page has worked on a variety of quality improvement projects in Grand Junction including the creation of a health information exchange, Docs on Call (an after-hours care setting), and the implementation of an electronic frailty index. Dr. Page serves as Adjunct Research Professor for studies on aging with Colorado Mesa University’s Center for Health Innovation and Practice and heads the community-based Frailty Workgroup.
Dr. Page’s practice, Western Colorado Physicians Group serves 12000 patients, where Dr. Page has been continually focused on improving the care of his patients. He spent over eight years on the board of Quality Health Network and was the Chair of their Quality Improvement Committee.
Patrick Page has traveled to England for a site visit with Dr. Andrew Clegg, who led the development, validation, and implementation of the award winning and NICE-recommended electronic frailty index in England. When Dr. Page isn’t seeing patients or working on frailty, he can be found mountain biking or skiing across Western Colorado.
