Matt Gannon is with the Primary Care Information Project (PCIP) at the NYC Department of Health and Mental Hygiene. PCIP supports a network of 21,000 New York City providers to develop population health capabilities. Currently, Matt leads PCIP's team that facilitates 300 practices' transformation to the New York State PCMH model. The NYS PCMH model was collaboratively developed by NYS and NCQA and provides a gold-standard roadmap for practices to deliver maximum value to patients, families, payers, and partners. NYS PCMH transformation is a foundational component of NYS's Roadmap to Value-Based Payment, which has set the ambitious goal of tying 80% of Medicaid primary care services to value by 2020.

Since 2013, Matt has consulted with hundreds of primary care practices of all sizes - his favorite challenge is to help stand up sustainable population health management programs within REALLY resource (people, money, IT, etc.)-strapped organizations. (Think: lots of Excel magic and even more finger-crossing). Previously, he was an Implementer at Epic. Matt holds a Bachelor's degree from Ramapo College of New Jersey.
