Joseph E. Fojtik MD, FACP, PCMH CCE

Joseph E. Fojtik, M.D., FACP, PCMH CCE attended the University of Illinois College of Medicine and completed an Internal Medicine Residency at Northwestern University. He has been in clinical practice for almost 30 year and currently also is a medical coordinator for the Illinois Deprtment of Fiancial and Professional Regulations. Previously the Medical Director of a multi-site PCMH initiative, he is now one of the NCQA PCMH Congress Steering Committee, a memebr of the NCQA Review Oversite Committee and one of the contributing authors for the NCQA CCE Digital Community.
