The University of Illinois at Chicago College of Pharmacy is accredited by the Accreditation Council for Pharmacy Education (ACPE) as a provider of continuing pharmacy education. Pharmacists have the opportunity to earn a maximum of 4.25 contact hours (0.425 CEUs) for participating in all knowledgebased activities at this conference. Attendance at each knowledge-based activity, completion of the selfassessment questions on the evaluation form, and submission of NABP eProfile ID and MMDD are prerequisites for receiving continuing pharmacy education credit(s). All ACPE credit awarded to participants must be uploaded to CPE Monitor™ within 30 days of the activity date, so pharmacists are required to provide their e-Profile ID and date of birth (MMDD) at the time of the activity.

In regard to successful completion and contact hours being awarded: 4.25 Continuing Nursing Education contact hours will be awarded to participants who attend the entire duration of the activity, and complete and return the evaluation form at the end of the activity. Advanced practice nurses may count a total of 3.75 contact hours for the pharmacotherapeutic educational requirement.
