Overview:
Presents an in-depth view of the current state of diabetes, including epidemiology and the expected increases in the prevalence of the diseases as the population ages; the importance of screening; differential diagnosis of diabetes; new and emerging treatments; patient engagement and practical strategies to implement patient-centered care in the treatment of diabetes.

Learning Objectives:
• Follow evidence-based guidelines from the American Diabetes Association for the care of patients with diabetes.
• Recognize the importance of treating to patient-centered goals and act to intensify therapies when warranted.
• Identify patient, physician, and systemic barriers to medication use and act to alleviate these barriers.
• Explain the importance of physical activity, weight management, and healthy lifestyle choices in the management of diabetes.
• Design interventions for diabetes that reduce cardiovascular risk factors by addressing blood glucose, blood pressure, lipids, and tobacco use.
• Implement diabetes self-management plans, patient education, and patient engagement tools to involve patients in their care.
