Discharge and care plans…are they ready to go & where do they go is organized by Appalachian Way Counseling, LLC. This course has been approved for a maximum of 7 Hours.

Overview:
This training is relevant to certification or recertification for educational requirements and professional growth issues, along with treatment planning, case management, and discharge.

This class provides an overview to address discharge planning in treatment and define characteristics, and risk factors with a focus on where and when the client is ready to go… Professionals will explore and discuss care planning components:
• SNAP- strengths, needs, abilities, and preferences
• Interpretive summary
• SMART goals
• Intervention plans and partners
• Discharge plans
• Role of staff, family, client and others

This class will provide an opportunity to develop discharge care plans and learn about valuable recourses.
