Mark Graber, MD, is a national leader in the field of patient safety and originated Patient Safety Awareness Week in 2003, an event now recognized internationally. Dr. Graber has also pioneered efforts to address diagnostic errors in medicine, and his research in this area has been supported by the National Patient Safety Foundation and the Agency for Healthcare Research and Quality.

Dr. Graber founded the Diagnostic Error in Medicine Conference series in 2008, the Society to Improve Diagnosis in Medicine in 2011, and the new journal, Diagnosis, in January 2014. In recognition of his contributions to bringing diagnostic quality and safety to greater attention, he received the John Eisenberg Patient Safety Award from the National Quality Forum and The Joint Commission in 2014. He has an extensive background in biomedical and health services research, with more than 80 peer-reviewed publications. Dr. Graber is a Professor Emeritus of Medicine at the State University of New York at Stony Brook.
