Medically high-risk patients with conditions such as obesity, heart and lung disease present as a challenge for the provision of office-based anesthesia. Coexisting illnesses also can complicate the provision of anesthesia in these groups. Few guidelines exist on the management of these special high-risk groups of patients for office anesthesia. This lecture specifically addresses the concerns related to maintaining the airway, ventilation, choice of anesthetic agents, factors related to the physiology of drug metabolism and problems related to anesthesia provision in this group of patients.

Learning Objectives:
At the conclusion of this program, participants should be able to:

• Describe the various medical/social conditions that present as a challenge to provide office anesthesia. 
• Demonstrate appropriate anesthesia techniques for the described high-risk conditions. 
• Discuss current preoperative and preoperative drugs to be considered for safe provision of anesthesia.
