New Models of Care: Diabetes and the Triple Aim is organized by American Association of Diabetes Educators (AADE) and will be held from Aug 06, 2017 - Aug 06, 2019.

The target audience for this medical event is designed for individual or groups of diabetes educators, including RNs, RDs, Pharmacists, Nurse Practitioners, Clinical Nurse Specialists, Physician Assistants, MDs, and other health care providers interested in staying up to date on current practices of care for their patients with diabetes and other related conditions. 

Accreditation Information

Registered Nurses:
The American Association of Diabetes Educators is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center's Commission on Accreditation. 

This educational program will provide 1.5 contact hours of continuing education credit.

Registered Dietitians:
The American Association of Diabetes Educators, (AM001) is a Continuing Professional Education (CPE) Accredited Provider with the Commission on Dietetic Registration (CDR). Registered dietitians (RDs) and dietetic technicians, registered (DTRs) will receive 1.5 continuing professional education units (CPEUs) for completion of this program. Continuing Professional Education Provider Accreditation does not constitute endorsement by CDR of a provider, program, or materials.

Registered Pharmacists:
The American Association of Diabetes Educators is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. This program provides 1.5 contact hours (0.15 CEU) of continuing education credit.

Program Overview:
Healthcare delivery is in the midst of a revolutionary change, driven in large part by a change in payment models. This shift in reimbursement is resulting in reorganizing healthcare to improve outcomes and reduce healthcare costs. This is good news for us in the world of diabetes because complication prevention is of high value. Diabetes educators have a critical role in improving diabetes outcomes and an opportunity to embrace care models to increase their value to the health system. We will discuss in an open forum the potential roles for diabetes educators in care management, practice coaching, and expansion of their services.

Course Objectives:
• Understand the role of reimbursement changes and their potential impact on care
• Describe new roles for diabetes educators in a value-based system
• Understand key aspects of population management, risk stratification, and care management
