Description:
The overall goal of this activity is to provide online tools and virtual practice improvement support to primary care practices in communities of color hard hit by COVID-19, so as to improve protection and health in those communities.
Learning Objectives:
Upon completion of this activty participants will be able to;
• Understand current COVID-19 trends.
• Learn practical tools for implementation in clinical practice to help prevent and treat COVID-19 in patients with chronic diseases.
• Delineate the relationship between COVID-19 and diabetes.
• Gather practical tips for optimizing chronic disease management, protection from, and treatment for COVID-19 in patients with diabetes.
