This presentation will explain the basics of why preparing for the end of life is important. It will also provide some essential resources for assessing needs and developing actionable plans. It will address the difference between palliative care and hospice and describe the value and necessity of creating an effective support team in serious illness and end of life care.

• Know how to access local resources concerning end of life and death preparation;
• Be able to explain the value of a skilled care team;
• Be able to facilitate the creation of a plan for end of life for themselves, the people they serve and/or loved ones
