Shira Shavit, MD, is an Associate Clinical Professor of Family Medicine at the University of California in San Francisco and the Executive Director of the Transitions Clinic Network. For over a decade, Dr. Shavit has been redefining national best practices for the care for individuals and communities impacted by incarceration.
She currently directs the Transitions Clinic Network, a national network of community clinics which specialize in providing whole person care to people with chronic health conditions who are returning from incarceration. To support this goal, the Transitions Clinic Network trains and employs people with a history of incarceration as community health workers, creating opportunities for people with a history of incarceration to leverage their experience toward a career in the healthcare setting. The Transitions Clinic Network received the Centers for Medicare & Medicaid Innovations Award in 2012 to expand the program nationally, and it has since been implemented in 15 community clinics in 7 states and Puerto Rico.
