Jonathon Hope BA

Currently, Jonathon is co-chairing ‘Realising the Value of Individuals’ a project funded by NHS England designed to trial the Patient Activation Measure (PAM) and to assess the effect of innovative, supportive approaches to helping individuals in their own self care journey. Over the last 10 years, Jonathon has held a number of leadership roles in healthcare improvement programmes nationally and across South East London. He is passionate about helping healthcare practitioners fully engage patients as genuine partners. His aims is for all patients to be empowered to be involved in their own care, where appropriate, and for their treatment plans to be aligned with their actual needs.
