Description:
Social Determinants of Health (SDOH) are conditions in the places where people live, learn, work, and play. This activity defines and explores the many dimensions of SDOH and the effects of SDOH on population health, how they inform your patient encounters and medical decision-making, and the connection between SDOH and health outcomes.

Learning Objectives:
At the conclusion of this activity, participants should be able to:

  • Define social determinants of health (SDOH)
  • Discuss the effects of SDOH on population health
  • Apply SDOH into clinical practice encounters and medical decision-making Recognize the relationship between SDOH and health outcomes
