Beta thalassemia is a rare genetic disorder characterized by impaired hemoglobin synthesis and chronic anemia, historically requiring lifelong transfusion and iron chelation protocols. While these conventional treatments are life-saving, they impose a heavy time, treatment, and quality-of-life burden on patients. With gene editing therapies and a rapidly expanding pipeline of novel disease-modifying agents, the multidisciplinary team faces a new clinical landscape. These breakthroughs offer the potential for reduced transfusion burden or even transfusion independence, but they bring complex considerations regarding regimen individualization, lifelong care planning, and long-term safety (Njeim et al, 2024; Hu et al, 2024). To optimize care from childhood through adulthood, the multidisciplinary team must be equipped to personalize treatment for patients using shared decision making. This Interactive Case Consult features Hanny Al-Samkari, MD, the Peggy S. Blitz Endowed Chair in Hematology/Oncology and the Co-Director of the Hereditary Hemorrhagic Telangiectasia Center at Massachusetts General Hospital, and Sherif Badawy, MD, MS, MBA, the Director of the Comprehensive Adult and Pediatric Thalassemia Program at Lurie Children’s Hospital of Chicago, Northwestern University Feinberg School of Medicine, as they explore evidence-based strategies for reducing treatment burden and transforming outcomes for patients with beta thalassemia.
