Are you providing the right level of care at the right time? How do you recognize when it’s appropriate for a patient to transition to another care level? This Webinar will examine a hospice’s responsibility to provide all four levels of care, and discuss how CMS utilizes data analysis to identify hospices that are falling short. In addition, faculty will explore case examples, provide strategies and discuss an audit tool to help you determine when general inpatient or continuous home care is best for the patient to keep you in the safe zone.
