Overview :
The goal of this activity is to help healthcare providers improve clinical outcomes for patients with heart failure. The discussion will focus on the integration of evidence-based recommendations, effective transition of care strategies across the continuum of care, and the optimization of pharmacologic therapies for patients with heart failure.
Learning Objectives :
• Identify and mitigate barriers to providing optimal care for patients with heart failure
• Utilize recommendations based on efficacy and safety, to co-manage patients with heart failure
• Implement strategies to enhance the utilization of transitional care teams in order to improve patient-centered outcomes in heart failure
