Description:
"Long-term Health Management for Adult Survivors of Pediatric Cancer" is planned and implemented interprofessionally by public health researchers, along with clinical practitioners in pediatric oncology and cancer survivorship care, with input from patients and community advocates. This activity is designed for primary care physicians, but may be of interest to nurses, nurse practitioners and social workers. This one hour course will provide education through lectures and case presentations. The activity will highlight best practices for managing long term health and wellness of adult survivors of pediatric cancer in the primary care setting and provide resources for providers to assist patients to access additional care or address barriers. The goal of this activity is to enhance knowledge of relevant survivorship guidelines and best practices, as well as skills and strategies for accessing additional resources and assisting patients to address barriers to care.

Objectives:
At the conclusion of this activity, learners will be able to:

• Describe the importance of care for adult survivors of pediatric cancer
• Describe some common components and benefits of care through survivorship clinics
• Identify key barriers to access for survivorship care
• Understand the potential for financial toxicity and how it affects survivorship care
• Identify what long term and late effects are associated with pediatric cancer therapy for adult survivors
• Locate and access resources for survivorship care, including guidelines, recommendations and locations of survivorship clinics
