Interdisciplinary Topics on Lessons Learned in Community - Based Palliative Care is organized by National Hospice and Palliative Care Organization (NHPCO) and will be held on Dec 13, 2018.
The target audience for this medical event is Nurse and Physician. This CME Conference has been approved for a maximum of 1.0 AMA PRA Category 1 Credits.
The growing need of individuals living with serious illness heightens the need for palliative care, particularly in community settings. While there is substantial need for community-based palliative care, scaling access to palliative care has been limited by a predominately fee-for-service payment environment. Trends in delivery system reform that reward value over volume create opportunities to scale access to high-quality palliative care in a financially sustainable way. In this Webinar, participants will explore how hospice providers can provide community-based palliative care in a way that supports seriously ill patients and families and offers strategic value to the organization.
Objectives :
• Describe three reasons hospice providers should consider developing or expanding community-based palliative care programs
• Articulate the challenges of providing community-based palliative care in a fee-for-service payment system
• Discuss how trends in delivery system reform create opportunities for the expansion of community-based palliative care
