Activity Summary:
Despite well-established treatment guidelines, many patients with diabetes are not managed with optimally effective insulin replacement programs while hospitalized. This session offers patient case scenarios demonstrating how to safely initiate insulin therapy while managing the critical care obstacles to optimal management of the hospitalized diabetic patient. Comprehensive treatment strategies to assist in managing insulin treatment for patients are discussed and are available for the attendees to bring back to their own unit. Included in this education is the use of newer devices such as insulin pumps and pens, standard glucose monitors and continuous glucose monitoring devices, and how to interact with the consultative support of diabetic self-management education (DSME) programs, dietitians, diabetes educators and other clinical services intended to help improve patients' blood glucose control and reduce their risk for therapeutic complications.

Objectives:
• Discuss common obstacles to insulin initiation, treatment and adherence in the management of type 2 diabetes.
• Identify strategies for initiating individualized insulin replacement therapies in the patient with type 2 diabetes.
• Explain how to safely and effectively utilize basal/bolus insulin in the hospitalized patient.
