Activity Overview:
Severe veno-occlusive disease/sinusoidal obstruction syndrome (VOD/SOS) is associated with a mortality rate of over 90% by ≥day 100 following hematopoietic stem cell transplantation (HSCT) and is not a disease restricted to the transplantation setting. It may also develop in patients receiving chemotherapy or calicheamicin antibody–drug conjugates not associated with HSCT. In the transplantation setting, exposure to inotuzumab or gemtuzumab prior to HSCT increases the risk of developing VOD/SOS. Early and accurate diagnosis with consistent clinical best practice in treatment and management improves outcomes for adult patients with VOD/SOS. A number of agents have recently been developed and investigated for use in both prophylaxis and treatment of this condition. Despite these developments, there is no uniform consensus on the optimal strategy for managing VOD/SOS. Variability in diagnostic protocols and subsequent management practices, as well as new developments in the treatment of this condition, have resulted in existing and emerging clinical and educational gaps.
