Course Description:
This course identifies four key activities of clinical care related to GER and GERD for generalists and pediatric subspecialists: Diagnosis and Testing, Treatment, Referral, and Education, Follow-up and Communication. Improving these activities will help clinicians deliver individualized, high-quality care. Diagnosing and managing patients with GER/GERD can be challenging. Gastroesophageal reflux (GER) is common in pediatric patients and accounts for a large number of visits to primary care providers and sub specialists. In fact, gastroesophageal re?ux (GER) occurs in more than two-thirds of otherwise healthy infants and is the topic of discussion with paediatricians at one-quarter of all routine 6-month infant visits. In addition to seeking guidance from their paediatricians, parents often request evaluation by pediatric medical subspecialists. In addition to caring for infants and children with GER and GERD, the subspecialist can expect to be consulted by primary care providers for help in managing such cases. A strong relationship with referring physicians provides an opportunity for the subspecialist to provide education regarding the difference between GER and GERD, avoid unwarranted testing in those with GER, and understand the indications and limitations of investigations when considered appropriate.

Be familiar with the following:
• Diagnosis and Testing: Accurate diagnosis of GERD, recognition of red flag or warning signals, knowing when diagnostic tests are appropriate
• Treatment: Knowing when non-pharmacologic treatment is recommended and when and what pharmacologic treatment is recommended
• Referral: Know when to appropriately refer a patient to a pediatric subspecialist and what referral information is needed
• Education, Follow-up, and Communication: Provide anticipatory guidance and GER/GERD education resources to patients and family; follow-up to ensure treatment is successful, and communicate effectively with other care team physician members and the patient/family including maintaining a care plan
