Core Competency Module 3: Monitoring and Modifying Cancer Treatment Care Plans are organized by Hematology/Oncology Pharmacy Association (HOPA).

Course Description: 
This module demonstrates the pharmacists’ patient care process in the development and implementation of the monitoring associated with a cancer treatment care plan. These patient-specific monitoring plans integrate basic pharmacology knowledge and clinical data. Both during-treatment and post-treatment (i.e. survivorship) monitoring plans are discussed. Additionally, the module describes and demonstrates the information needed to effectively modify care plans based on the efficacy and toxicity of previous treatments. 

Learning Objectives:
• Identify the appropriate clinical laboratory and other data required to assess the effectiveness and safety of a cancer treatment care plan 
• Formulate appropriate patient-specific monitoring plans based on the evaluation of laboratory and other clinical data for initial and subsequent cancer treatments 
• Recognize drug-related and disease-related symptoms 
• Construct a modified cancer treatment care plan based on patient-specific laboratory and other clinical data 
• Propose a survivorship care plan given patient-specific information
