Description:
Hypoparathyroidism is a rare disorder characterized by decreased function of parathyroid glands resulting in low levels of parathyroid hormone (PTH). There may be a variety of causes that include autoimmune disease, congenital defects, unintended removal during thyroidectomy, or as a result of damage due to radiation therapy. In a normal functioning parathyroid gland, PTH is secreted in response to low serum-ionized calcium. Patients with hypoparathyroidism experience abnormal mineral homeostasis affecting bone, kidney, and the gastrointestinal tract. Pathogenesis of the condition includes reduced osteoclast-mediated bone resorption, reduced calcium reabsorption in the distal convoluted tubules of the kidneys, and increased phosphorus reabsorption from the proximal convoluted tubules, causing hyperphosphatemia. In severe, untreated cases, hypocalcemia can result in abnormal skeletal formation, cognitive impairment, bronchospasms, laryngospasms, neuromuscular irritability and cardiac issues.

• Describe the pathophysiology and underlying etiologies of hypoparathyroidism and apply these to patient cases.
• Describe the clinical manifestations of hypoparathyroidism and discuss how they may have relevance to treatment targets.
• Identify available therapeutic options for hypoparathyroidism and incorporate these in the development of treatment plans in patient cases.
• Define patient-specific goals related to the management of hypoparathyroidism.
