Statement of Need:
Neuroendocrine carcinomas (NECs) are a rare and highly aggressive group of tumors that can arise from neuroendocrine cells anywhere in the body. While the lung is the most common site of origin, approximately 9% are classified as extrapulmonary NECs (EP-NECs), which arise most commonly in the gastroenteropancreatic tract, followed by the genitourinary and gynecologic tracts (Stumpo et al, 2023; Dasari et al, 2017). Diagnosis of EP-NECs is complicated by their diverse clinical presentations, and there is suboptimal evidence for guiding treatment recommendations. Ongoing trials evaluating targeted therapies, immune checkpoint inhibitors, and radionuclide therapy are showing promise in the treatment of EP-NECs, and remaining up to date on emerging evidence and multidisciplinary strategies is essential to improving patient outcomes (Stumpo et al, 2023). In this Video Viewpoint Strategy Session, Andrew E. Hendifar, MD, Associate Professor of Medicine and Medical Director of Pancreatic Cancer at Samuel Oschin Comprehensive Cancer Center of Cedars-Sinai Medical Center, and Danielle Hutchings, MD, Assistant Professor of Pathology and Laboratory Medicine at Cedars-Sinai Medical Center, will provide expert insights on diagnosis, treatment, and emerging targets in EP-NECs.

Learning Objectives:
Upon completion of this activity, participants should be able to

Differentiate the clinical and pathological characteristics between neuroendocrine tumors (NETs) and NECs
Apply key diagnostic criteria for evaluating EP-NEC to minimize misdiagnosis and diagnostic delays and facilitate timely initiation of appropriate treatment
Outline evidence-based treatment strategies for patients with EP-NEC, including first-line therapies and approaches for progressive or relapsed disease
Evaluate the role of DLL3 as an emerging therapeutic target in EP-NEC, including the rationale for its use and supporting evidence.
