Cancer Documentation and ICD-10-CM Coding_Enduring is organized by UPMC Center for Continuing Education in the Health Sciences (CCEHS).

Description:
During this presentation, we will provide a comprehensive overview of official documentation and coding guidelines needed for proper cancer reporting, focusing on:

  • “History of” documentation.
  • Definitions of active disease/treatment versus monitoring for recurrence.
  • Solid tumors, lymphoma, and leukemia.
  • Primary versus secondary (metastatic).
  • Documentation examples for proper ICD-10-CM code assignment.

Learning Objectives
Upon completion of this activity, participants should be able to:

  • Demonstrate a comprehensive understanding of official documentation and coding guidelines for proper ICD-10-CM cancer code reporting, based on the criteria of active treatment/active disease versus monitoring for recurrence.
  • Build skills of best practice documentation for solid tumors (primary and metastatic), lymphoma, and leukemia.
  • Improve upon determining whether an active cancer code or a “history of” cancer code should be reported, based on provider documentation examples.
