Description:
This webinar is the third in the four- part webinar series Heart Failure Best Practices. In this 3rd webinar of the series esteemed faculty will discuss consensus-based and evidence-based methods of ensuring effective care coordination, transitions and optimization of heart failure management by the healthcare team. 

Learning Objectives:
At the completion of this course, the learner will be able to:
• Achieve benchmarks for national heart failure quality measures, including appropriate diagnosis, evidenced-based treatment of acute and chronic heart failure, reduced hospital readmissions, and patient-centered clinical outcomes.
• Apply current evidence-based guidelines and expert consensus approaches to making individualized treatment decisions regarding conventional and new therapy for acute and chronic heart failure.
• Apply consensus-based and evidenced-based methods for ensuring effective care coordination to improve quality and clinical outcomes in heart failure.
