Ah, serum ammonia. A lab that is often sent and often a head-scratcher. Sometimes an altered patient with cirrhosis and the most textbook asterixis will have a normal serum ammonia. And other times a patient has a sky-high ammonia and you have no idea why. Have you ever stopped and checked if they’re on valproic acid?

Objectives:

Upon completion of this activity, you will be able to:

  1. Generate hypothesis for how valproic acid may lead to elevated serum ammonia.
  2. Review the possible treatment options for valproic acid associated hyperammonemia.
  3. Describe the evolution of scientific and clinical understanding of valproic acid–associated hyperammonemia, including key historical developments and changes in diagnostic or treatment approaches.
