Description:
Join us for a live webinar designed to help PCMH practices understand the larger goals of Annual Reporting and prepare for annual submission for reporting the year 2021 under the Patient-Centered Medical Home program.

Faculty will highlight ways to demonstrate sustained recognition status through maintenance and continuous quality improvement efforts using data that a practice is already collecting internally and for other sources (e.g., state initiatives). Timelines for deliverables and performance expectations will be key discussion items.

At the conclusion of the webinar, participants will be able to:
• Outline the Annual Reporting requirements for PCMH recognition.
• Describe the Annual Reporting process; including timelines, submission and audit.
• Interpret Annual Reporting requirements for reporting year 2021.
• Recognize important features and functions of Q-PASS to submit for Annual Reporting.
