Description:
This session covers how to conduct, score, and interpret a cognitive screening tool, brain health planning, billing for screenings, and documenting a patient’s healthcare agent. 

Educational Objectives:
Upon completion of this activity, participants should be able to:

  1. Discuss the importance of conducting, scoring, and interpreting a cognitive screening tool (e.g. Mini-Cog or GP-Cog)
  2. Develop a brain health plan with patients after conducting a cognitive health assessment
  3. Explain how to bill Medi-Cal using code 1494F for Medi-Cal-only beneficiaries ages 65 and older
  4. Document a patient's health care agent in their medical record as part of the cognitive health assessment
