Description:
For more than 25 years the Academy has represented the forefront of medical care in the home, and this year we will explore the transforming landscape of Home-Based Medical Care. Increasingly, primary, palliative, hospital, urgent, and interdisciplinary care have moved to the home. The pandemic has only reinforced this need and accelerated the trend.

Learning Objectives:
• Explore innovative models of care, learn evidence-based best practices, discover new strategies to improve care coordination through interdisciplinary care, and acquire the skill set to implement rapid performance improvement at this year’s conference.
• Describe how to manage complex clinical care in the home.
• Explore opportunities for innovating and growing home-based medical care programs.
• Identify quality measures that are important to track, as well as approaches to quality improvement, to demonstrate value to clinicians, patients, families, and payers.
• Explain the need for clinical leadership and adaptability in an ever-changing health care environment.
• Identify ways to integrate health equity into day-to-day practice and be more inclusive of underserved populations in home-based care programs.
• Describe how to begin to transition into population health and value-based care.
