Henry Ford Physician Network Summit: Collaborating for the Future

The Henry Ford Physician Network ("HFPN") is a physician-driven subsidiary of Henry Ford Health System ("HFHS") comprised of Henry Ford Medical Group (HFMG) system-employed and private practice physicians. Anticipating health care reform legislation and leveraging the longstanding collaboration among its members, this clinically integrated network of 1600+ physicians was incorporated in March 2010. The HFPN strategy includes:

1. Standardizing care based on leading practice;

2. Using technology to share clinical data and improve care coordination and continuity;

3. Establishing uniform performance expectations, metrics, targets and thresholds; and

4. Developing common goals and reward structures.
