Tang Woon Hoe started the NKF PD Community Support Programme with a team of nurses in 2011. Her work focuses on enabling PD choice by closing the PD support gap in the community in partnership with the hospitals. The Programme provides patient-centred, comprehensive and structured home-based PD support which is warmly received by the patients as well as the PD programmes in hospitals. The Programme won the ILTC good practice award in ILTC Excellence Award 2014 and Best Team Merit award in SHQSA 2018.
Ms Tang has a Bachelor degree in Nursing and advanced diploma training in Nephro-urology. Her PD experience started when she was working in the National University Hospital in 1997. She left NUH to be a Nursing lecturer in Institute Technology of Education in 2006. She joined the National Kidney Foundation in 2008 and is currently the assistant director of Nursing, overseeing PD community support Programme in NKF. She is an active member of the MOH work group participating in the planning and implementation of the National Home PD Support Programme.
