Sarah J. Crane, M.D., studies system-based support of specific populations in primary care practices. Areas of emphasis include the development and utilization of administrative risk stratification scores for frail, older adults with the goal of directing resources to support those patients most at risk of adverse consequences, such as hospital admissions or readmissions. Additionally, Dr. Crane has studied multiple new models of care such as electronic online patient communication and how data can be used to direct resources into new models of virtual care.
