Portia Buchongo RN, MPH, PhD

Portia Buchongo, RN, MPH, served as the Hypertension and Diabetes Nurse Consultant for the Chronic Disease Section of the Georgia Department of Public Health, where she led the hypertension and diabetes nurse protocol committees and promoted the use of team-based care and evidence-based strategies for diabetes and hypertension prevention and control across the state. Previously, she was an ORISE Fellow at the Centers for Disease Control and Prevention, where she was responsible for program evaluation and served on the State Innovation Model workgroup.
