Phillip Farwig PharmD, BCACP, TTS

Phillip Farwig currently works as a clinical pharmacist in a primary care setting at The Ohio State University Wexner Medical Center’s Division of Family and Community Medicine. Before his current job, he trained within the Veteran’s Affairs system as an ambulatory care pharmacist working in various direct patient care clinics such as diabetes, hypertension, lipids, anticoagulation, hepatitis c, home base primary care, and endocrinology. His training at the VA provided me with the solid clinical backbone that he currently brings to his healthcare team and their patients. He loves to work with chronic disease state management, population health, and drug information. He has a particular interest in and passion for diabetes management. Through these skills, He strives to empower patients by providing them with the knowledge and guidance required to optimally manage their chronic conditions.
