Pamela (pam) Lagman Mendoza PharmD

Dr. Mendoza is a transition of care pharmacist dedicated to improving patient care as they move from the inpatient hospital setting to home or other care settings. She collaborates with physicians, nurses, and other allied health professionals to assess the appropriateness of drug therapies and ensuring that they are affordable to patients to prevent any interruption in therapy. She has a special interest in improving medication reconciliation, addressing medication adherence barriers, and providing patient education to promote safe medication management for high-risk patients.
