Nicole Lynn Hahn PharmD, BCACP

Nicole Hahn is a clinical pharmacy specialist in Neurology with Kaiser Permanente Colorado (KPCO). She received her Doctor of Pharmacy degree from Wilkes University in 2009, and completed a PGY1 pharmacy practice residency at Allegheny General Hospital in Pittsburgh, Pennsylvania followed by a PGY2 ambulatory care practice residency at University Maryland-Baltimore School of Pharmacy. She moved to Denver, Colorado in 2011 where she practiced in the clinical pharmacy cardiac risk service at KPCO. In 2013 she transitioned to her current role as clinical pharmacy specialist in Neurology where she supports an outpatient general Neurology practice. Her clinical practice includes managing disease-modifying therapies for multiple sclerosis through a collaborative drug therapy management protocol, providing consultative services to providers for all neurologic disease states (including epilepsy, migraine, movement disorders, etc), coordinating efforts for contraception counseling for women with epilepsy, along with population management and patient safety initiatives. She is a Clinical Instructor for the University of Colorado Skaggs School of Pharmacy and Pharmaceutical Sciences and a Clinical Affiliate Faculty at Regis University School of Pharmacy. In addition to precepting student pharmacists, Dr. Hahn works with PGY2 ambulatory care residents through the ASHP-accredited program offered at KPCO.
