Libby Hoy is Founder and CEO of Patient- and Family-Centered Care Partners, a resource organization on the West Coast committed to the shared learning of patient- and family-centered care (PFCC) practice. PFCC Partners established the PFANetwork of more than 150 patient family advisors working in health care settings across the country to partner for improvement in the quality, safety, experience, and delivery of health care. Ms. Hoy has 23 years of experience navigating the health care system as the mother of three sons living with mitochondrial disease.
