Leonard R. Hock, Jr., DO, MACOI, CMD joined Trustbridge following more than 20 years as President of an Internal Medicine practice dedicated to the care of frail elderly, specialized geriatric care, and hospice and palliative medicine. Dr. Hock developed a palliative case management model for acute care hospitals that have proven to be successful in decreasing the length of stay and acute care costs while increasing the comfort measures for the end-of-life patient and support to family members. During his medical career in Kansas City, he was the health care reporter for FOX 4 Television News. For thirteen years he was seen daily on “Housecalls” by thousands of viewers who watched him speak on current and pertinent health topics.

He is an excellent public speaker and advocate of hospice and palliative care within the community. He continues to be a frequent local, regional and national presenter on medical, scientific, and cultural issues. A Veteran of the United States Army, Dr. Hock held several Physician Executive positions prior to his appointment at Trustbridge. He is Board Certified in Internal Medicine, Geriatrics and Hospice and Palliative Medicine.

He is also a Diplomat of the American Board of Hospice and Palliative Medicine and a Member of the American Academy of Hospice/Palliative Medicine. In his role as Chief Medical Officer of Harbor Palliative Care and Advanced Illness Management of Trustbridge, Dr. Hock has concentrated his efforts on building our physician team and expanding our contracted physician network with much success. He was elected to the Board of the Florida Medical Directors Association and appointed Chair of the Hospice subsection.
