Dr. Kathryn Santos is Associate for Quality Improvement Initiatives at the Continuing Care Leadership Coalition (CCLC), which is the long term care affiliate of the Greater New York Hospital Association (GNYHA). With over 15 years of experience working on clinical long term care issues in the in patient and community-based settings, a personal commitment to person-centered care, and a strong background in performance improvement and project management, Ms. Santos is the point person for the facilitation of quality improvement policy, strategy, and projects on behalf of nearly 100 CCLC member nursing home and home health care providers, and represents CCLC and its members on quality-related matters at the State level and in national quality improvement campaigns.

While at CCLC, Ms. Santos has co-authored a Journal of the American Medical Directors Association (JAMDA) publication entitled, Reducing Potentially Preventable Hospital Transfers among Nursing Home Collaboratives.

Prior to CCLC, Ms. Santos assumed clinical and administrative responsibilities and roles as Director of Rehabilitation, Health Policy and Management Consultant, Rehabilitation Consultant, and Physical Therapist.

Ms. Santos holds a Bachelor's Degree in Physical Therapy and a Master's Degree in Health Policy and Management.
