Jordan A. Wolf PharmD, BCGP

Jordan Wolf is a Long Term Care Consultant Pharmacist for Thrifty White Pharmacy, where he has been employed since 2012. He graduated from the North Dakota State University College of Pharmacy, Nursing, and Allied Sciences in 2012. He successfully obtained his Board Certification in Geriatric Pharmacy in November 2014. He currently serves as President-Elect for the Minnesota Chapter of the American Society of Consultant Pharmacists.
Through his current role as a consultant pharmacist, Jordan has extensive experience in medication management in a variety of settings, including community, Skilled Nursing Facilities, Assisted Living Facilities, group homes and correctional facilities. He is a certified trainer for the American Pharmacists Association’s MTM Certification Course, and has taught this course on behalf of MPhA on multiple occasions.
