Jenna Mayer MPH

Jenna Mayer is a Project Specialist and Patient-Centered Medical Home Consultant at Partners HealthCare, where she supports primary care practices as the network works toward the highest level of recognition from the National Committee of Quality Assurance. She is also Project Manager for the implementation of social determinants of health screening across the Partners HealthCare primary care network.
