Jannet M. Carmichael PharmD, FCCP, FAPhA, BCPS

annet M. Carmichael, Pharm.D., FCCP, FAPhA, BCPS is the VISN 21 Pharmacy Executive for the VA Sierra Pacific Network covering Northern Nevada, Northern California and Hawaii. Dr. Carmichael coordinates a Health Informatics, Pharmacoeconomic and Pharmacoepidemiology program using a Clinical Data Warehouse and PBM performance metrics for Veteran Integrated Service Network (VISN 21). Prior to this she was Clinical Pharmacy Coordinator at the VA Sierra Nevada Health Care System in Reno, Nevada and Associate Professor of Medicine at the University of Nevada School of Medicine. Her clinical practice includes primary care and outcomes studies.

Jan is a past-President of the American Society of Health-System Pharmacists (ASHP), winner of the 2013 ASHP Whitney Award, past member of the Nevada Board of Pharmacy, Past-President of the Nevada Pharmacists Association and of the Nevada Pharmacy Alliance. She participates as a member of numerous professional boards and committees. She is past-Chair of the Board of Pharmaceutical Specialties, has Chaired the BCPS Pharmacotherapy Council and is an ACPE Practitioner Reviewer. Jan served as a member of the National Quality Forum (NQF) Medication Management Sterring Committee and was a member of the CMS Medication Measures Technical Expert Panel. Jan has authored hundreds of professional papers, book chapters and research projects. She is a frequent speaker on progressive pharmacy practice throughout the country.

Reno, Dr. Carmichael taught at the University of Iowa, College of Pharmacy and the University of North Carolina, College of Pharmacy. Dr. Carmichael is a graduate of the University of Iowa College of Pharmacy and the University of the Pacific College of Pharmacy Pharm.D. program. She is a Board Certified Pharamcotherapy Specialist, a Fellow of the American College of Clinical Pharmacy and a Fellow of the American Pharmacist Association.
