Caroline S. Blaum MD

Dr. Caroline S. Blaum is the associate medical director of the Faculty Group Practice and the assistant dean for clinical affairs at the University of Michigan Medical School (UMMS). She is also a research professor at the Institute of Gerontology of the University of Michigan Health System (UMHS), a professor of internal medicine and geriatrics at UMMS, and a research scientist at the Ann Arbor VA Geriatrics Research, Education, and Clinical Center. Her research concerns models of care, disability, complex patients, frailty, and diabetes in older adults. She sees patients in ambulatory, acute, and sub-acute settings and is active in teaching internal medicine and geriatrics to medical students, house officers, and geriatrics fellows.

She leads the UM Health System's Population Health Program, including its activities in the Pioneer Accountable Care Program and the Michigan Multipayer Advanced Primary Care Demonstration. From 2005 to 2010, she led the UM Health System's successful effort in the Physician Group Practice Medicare Demonstration that demonstrated a reduction in Medicare growth along with the improved quality of clinical care for Medicare beneficiaries and was a key model for current accountable care organization policy. Dr. Blaum was also the principal investigator of the Ann Arbor VA Healthcare System's Patient Aligned Care Teams Demonstration Laboratory, a project evaluating transitional care interventions that focus on caregiver support and self-management support, as well as care coordination and primary care redesign consistent with advanced primary care practice.
