Carol Cianfrone RN

Carol Cianfrone is a Senior Director of Medicaid Managed Care Programs, has been with HNJH since 2013. Since joining Horizon, Carol played an integral role in the development and implementation of Horizon’s Managed Long-Term Services and Supports program. She now oversees the day-to-day operations of all of the Horizon Medicaid Care Management Programs which encompasses over 600 clinical and non-clinical staff.

Carol has extensive experience in caring for the underserved and managing highly complex populations, including dual-eligible and long-term care recipients. She has worked directly with patients, and as part of multidisciplinary teams across varied medical settings, including Emergency Rooms, Home Care, and Nursing Facilities, caring for both the pediatric and geriatric populations. Ms. Cianfrone received a Bachelor of Science degree in Nursing from Trenton State College (The College of New Jersey).
