Community Health Partnership, Inc. (also known as “The Partnership”) was founded in 1993 in response to community concern for the viability of community-based, primary care clinics. By joining together in a local consortium, community health centers were able to enhance their services to low-income patients through the Partnership’s collaborative projects. Initially serving Santa Clara County clinics, the Partnership expanded into San Mateo County in 2004.Community Health Partnership is a consortium organization composed of community health centers. We believe in affordable, accessible, and quality patient-centered health care for all of Santa Clara and San Mateo Counties ’ residents.The Community Health Partnership is one of seventeen membership organizations throughout the state of California that represent non-profit Community Health Centers. We are comprised of eleven primary care community‐based organizations providing health services at 31 sites, including five school health centers, three free clinics, an Indian Health Center and seven family planning clinics. The clinics combined serve 144,659 patients per year with 417,051 patient visits annually.
